To answer your question, it did indeed originally come from the game on All the Fallen, which was archived a long time ago. I just realized they've recently released a more up-to-date version, which wasn't available when I downloaded it. So, if you want the new version, check the forum. Otherwise, the MediaFire version, although outdated, still works.
