To use "underground healer" misses the point of being illegal. It carries the connotation of being dodgy, secluded, or independent- which tickles at meeting the other meaning of yami but doesn't quite reach lurking the underworld so much as making him out to be a hipster faggot with the latest hottest healing techniques. Personally I don't remember what you would call a doctor's office working on the black market but surely it's not underground clinic.
